NOTIS PENGEMASKINIAN BORANG APPLICATION FORM FOR IMPORTATION OF FEED INTO MALAYSIA (PET FOOD / ANIMAL FEED / FEED ADDITIVE CONTAINING ANIMAL-BASED RAW MATERIALS)

Pemakluman ini adalah bertujuan untuk memaklumkan pengemaskinian borang “Application For Importation Of Pet Food / Animal Feed Into Malaysia” untuk tujuan kelulusan pengimportan makanan haiwan yang mempunyai bahan mentah berasaskan haiwan. Borang yang dikemaskini adalah seperti di lampiran di bawah.

Pengemaskinian di dalam borang ini melibatkan keperluan verifikasi pihak berkuasa negara di mana makanan haiwan ini telah diproses. Langkah ini diambil berikutan kelulusan pengimportan adalah berdasarkan semakan dokumen. Justeru, adalah penting supaya maklumat yang diberikan telah disahkan sebagai benar oleh pihak berkuasa yang memantau operasi loji pemprosesan makanan haiwan tersebut.

Borang permohonan boleh didapati di laman web www.dvs.gov.my > EBorang > Import & Eksport. Penggunaan penuh borang ini akan berkuatkuasa penuh pada 1 Julai 2024. Pihak berkuasa daripada negara pengeksport yang memerlukan klarifikasi boleh menghubungi DVS melalui email di skie@dvs.gov.my .

NOTICE ON THE UPDATE OF APPLICATION FORM FOR IMPORTATION OF FEED INTO MALAYSIA (PET FOOD / ANIMAL FEED / FEED ADDITIVE CONTAINING ANIMAL-BASED RAW MATERIAL)

This is to inform that there has been changes to the “Application Form For Importation Of Feed Into Malaysia” for the import authorisation of pet food/animal containing animal based raw materials. Please find the updated form as in below.

The updates in the application form includes verification by competent veterinary authority of the country where the feed was manufactured. This measure has been taken as the approval of import of pet food/animal containing animal based raw materials is performed by document review. Thus, it is important that the information provided has been endorsed to be true by the competent veterinary authority which supervises the operation of the said feed manufacturing establishment.

The form is retrievable at the website www.dvs.gov.my > E-Borang > Import & Eksport. The use of the form will be fully enforced by 1 July 2024. The competent authority from the exporting country requiring further clarification may contact DVS for further clarification at skie@dvs.gov.my .
